Greetings!! My name is Chanda Hazelwood, and I am pleased to meet you. I have worked in the mental health field for over 15 years specializing in the psychosocial rehabilitation of youths and adults. I have experience in providing inpatient, in-home, and in community care as well as providing targeted case management services.
My goal is to help you and/or your loved ones achieve mental and social stability in a setting that is both safe and supportive.
We are excited to welcome Dr. Melissa Frank, Psy.D., to Renaissance Behavioral Health, LLC as a Qualified Mental Health Professional. Dr. Frank brings over 15 years of diverse experience in health coaching, organizational wellness, and behavioral health. With a Doctorate in Psychology emphasizing Health and Wellness, she specializes in person-centered care, motivational interviewing, and evidence-based strategies to promote positive behavioral outcomes.
Dr. Frank has served in various clinical, academic, and coaching roles. She is known for her warm, solution-focused approach, her ability to build strong therapeutic relationships, and her expertise in group facilitation and health education.
We are honored to have Dr. Frank join our team and look forward to the insight, energy, and compassion she brings to supporting our clients and community.
Renaissance Behavioral Health, LLC is proud to welcome Chaisa DuBrey to our team as a Qualified Mental Health Associate. With over 20 years of diverse experience in social services, case management, residential care, and behavioral health, Chaisa brings a wealth of knowledge and compassionate leadership to our organization.
Chaisa’s professional background includes pivotal roles such as Case Manager Lead at Jewish Family Service Agency, Residential Manager at ResCare-Educare, and Family Services Assistant with the Clark County Department of Family Services. She has overseen large teams, developed and implemented individualized service plans, conducted internal investigations, and ensured compliance with state regulations—all with a person-centered approach that empowers individuals and families.
Her academic accomplishments include a master’s degree in public administration and a Bachelor’s in Criminal Justice with a focus on Juvenile Justice.
Chaisa holds certification as a Qualified Intellectual Disabilities Professional (QIDP) and in CPR, First Aid, and HIPAA/OSHA compliance. She is highly skilled in behavioral support planning, quality assurance, intake coordination, and Medicaid-aligned documentation and care strategies.
